I am writing this letter to report an adverse drug reaction that happened to one of my customers at our community pharmacy. The customer's name is Daniel,78 years old with a history of recurrent hypertension. Mrs Daniel came to our pharmacy early today asking a treatment for skin rash,; the rash has been troubling Mrs Daniels for a few days. After thorough investigation, trying to acknowledge what is the cause of the rash, I discovered that 2 weeks ago MRSrs Daniels started taking a recently released ACE inhibitor which is called Drug X. Mrs Daniels's doctor prescribed Drug X after the patient started suffering from verapamil 's side effects previously. Drug X is mainly prescribed by doctors for treatment of hypertension and cardiac failure.
I advised Mrs Daniels to visit her family doctor as soon as possible, to avoid any further complications. Additionally,I called Mrs Daniels's doctor explaining to him all the details about Mrs Daniels's case. Dr J Sotto confirmed that he will put Mrs Daniel on otha different class of antihypertensive drugs. I have been informed that Mrs Daniels is currently using other medications to treat hypertension. The additional medications are nNitrazepam,v Verapamil and iIndapamide,. Mrs Daniels has been using them as follows: 5 mg once every morning, 80 mg twice a day and 2.5 mg three times a day respectively.
Mrs Daniels's rash was quite evidencet, covering large areas of her body, specifically the trunk and limbs. According to the patient, there were not any changes in her diet or laundry materials . I provided Mrs Daniels with immediate treatment to resolve her skin rash. The rash treatment was 5% hydrocortisone cream,; it should be applied 3 times daily.
