I’m writing to inform you that Mrs. Taylor is being discharged from our hospital in to your care today, 01/11/2018, after being treated fofor a fractured of pubic ramus and mild kidney injury .
Mrs. Taylor is an -85year old and85-year-old who lives alone.she She is able to mobilize independently and is scheduled to be seen by careerscaregivers twice a day.
She suffered a fall at home and started to complain of a painful on right hip and lower back pain and was enabledunable to mobilize her self. andShe was found in this situation by careerscaregivers.
Her past medical history is unremarkable, apart of Hypertensionfrom hypertension and osteoporosis.
During hospitalization, Mrs. Taylor initially received analgesia, then she was assessed by aan Fy2 doctor and was reviewed by a consultant.
At the time of admission, her examination was unremarkable aside from L3 lower back pain and pain in the right pelvic area.
Blood investigationtests were within normal range, except slightlyslight elevation in creatinine and urea.
X-rays showed that there was fractureda fracture of the inferior pubic ramus.
She was mobilized in the department by a physio and occupational therapy and was fully assessed by frailty teams alsoas well.
WithIn discussion with all medical teams and the next of Kinkin, her son Stephan Taylor, it was decided that Mrs. Taylor would be dischargedischarged to her home today, 01/11/2018, with support.
CareersCaregivers' visits were increased to visits four times a day from twice daily.
Her discharge medications includesinclude paracetamol 1gr QDS and codeine 30-60, as required.
I recommendrecommended that Mrs. Taylor to increase her oral fluidsfluid uptake and she was given a walking frame.
Please note., she needs to review her condition with her GP., including her kidney function test, in a weeksweek's time .
