I found the app to help us routinely monitor and assess for COVID-19 symptoms and risk factors in a safe and secure manner. I have done a lot of research and determined that Health Champion, is a secure app and will allowsallow us to collect and manage daily assessments.

You should be receiving an invitation from The Health Champion app to test the system and participate in the pathway screening feature. Please see below for instructions on how to log in and participate:

1. You should have received an email from Health Champion inviting you to join Falcon Investments Boston or NY Group.
2. Please download the app and join the group.
3. From the member app home screen, you can tap the pathway to view and complete the test.
